*WAIVER, INDEMNIFICATION AND ASSUMPTION OF RISK: I/We, the parents and or guardian of the above named candidate for a position on a league team, hereby give my/our approval to participate in any and all league activites. I/We assume all risk and hazards incidental to such participation, including transportation to and from the activities. I/We do hereby waive, release, absolve, indemnify and agree to hold harmless the London Youth Baseball League, the British Baseball Federation, Baseball/Softball UK, the chartering organization and all of its board members, officers, managers, officials and coaches, the sponsors of and the participants in baseball activities and all persons transporting my/our child to and from baseball related activities for any claim arising out of an injury to my/our child, whether the result of negligence or from any other cause, except to the extent and in the amount covered by accident and liability insurance. I/We understand that the insurance carried by this league covers only the amount that is not paid by my/our carrier. I/We will furnish a certified birth certificate of the above named candidate to league officials upon request. I/We hereby give permission to all LYBL officers, managers and coaches to authorize emergency care for the named candidate.

*MEDICAL: I/We certify that my child is in good physical condition, is fit to participate in the London Youth Baseball League Program and suffers from no physical impairment that might be aggravated by said participation. I certify that my child is covered by medical insurance and that I will not hold the London Youth Baseball League, the British Baseball Federation, Baseball/Softball UK, the chartering organization and all of its board members, officers, managers, officials and coaches, the sponsors of and the participants in baseball activities and all persons transporting my/our child to and from baseball related activities for any injuries or harm to my child which occurs during participation in the Little League program. I acknowledge the contagious nature of COVID-19 and that public health authorities in the United Kingdom still recommend practicing social distancing. I further acknowledge that the London Youth Baseball League has put in place preventative measures to reduce the spread of COVID-19, but cannot reasonably guarantee that a player or family member will not become infected with COVID-19. I understand that the risk of becoming exposed to and/or infected by the COVID-19 may result from the actions, omissions, or negligence of others, including, but not limited to, myself, other players, umpires, fans, and family members. I assume all risks associated with participating in baseball activities offered by the London Youth Baseball League. I will hold harmless the London Youth Baseball League, the British Baseball Federation, Baseball/Softball UK, the chartering organization and all of its board members, officers, managers, officials and coaches, the sponsors of and the participants in baseball activities and all persons transporting my/our child to and from baseball related activities for any illness, including exposure to COVID-19.
